Getting Started

As a medical professional, we understand your needs in regards to advanced testing.  You want specialized service, delivered efficiently.  We value your business and our promise is to do whatever we can to make the ordering process an easy and simple one.  Please follow the instructions below to set up an account and to begin ordering samples through our lab.

Follow these Steps to get started with Boston Heart Lab

1. Contact Client Services @ 508-877-8710 (then press 5) OR email clientservices@bostonheartlab.com with your contact information and a representative will contact you within 24 hours to create an account in our database.
2. Once you account is created, we will send out all necessary supplies, kits, prepaid labels and forms so you can begin ordering.  It’s that easy.
3. Pricing - Our advanced CV tests are covered by most insurances and Medicare patients will likely have no out of pocket cost.  For more details, please contact client services.

Required Forms when submitting Samples
Once you receive your kits, the following forms will be included.  Please feel free to download the forms listed here if you need more or feel free to contact our client services department and we will provide any necessary items needed.
a) Lab Requisition Form – Please complete all areas required on the form including appropriate diagnosis codes, patient billing information, tests ordered and signatures.  Incomplete information on the forms may delay the processing of the sample so please become familiar with the information before ordering.  With an appropriate diagnosis, most insurance companies provide coverage for our tests. Medicare patients will likeley have no out-of-pocket cost.
b) Patient Registration / Insurance Form – If ALL the information on this form is included separately with the sample, then this form does not need to be used but it is highly recommended since any missing billing information may delay the processing of the sample.  Remember to also include the FRONT/BACK copies of the patient insurance card.
c) Blue Cross / Blue Shield Waiver – ONLY for BC/BS patients, please have these patients review/sign the letter & include with the sample.
d) Instructions for Blood Draw - Phlebotomists need a refrigerator and centrifuge for preparing and storing blood samples and a freezer for ice packs.  Please do not draw/ship a day before a holiday as our offices will be closed.

Tests Results

1. Results are sent to the practice either by fax, mail or electronically.  We can also provide more than one copy of the test report if required.
2. Each patient will also receive the Navigator, a custom patient education book to take home with them which also includes a copy of the test results.
3. Interpreting Results – Our clinical specialists are can answer any questions you may have.  Please email medicalservices@bostonheartlab.com if you have any specific questions OR call Client Services & they will direct you to a field representative near you.
